To enter the Trial, complete the 2010 Ontario Regional Championship Entry Form.

Reminder - deadline date for submitted trial entries is
May 3, 2010.

To enter the Show and Breed Survey, complete the 2010 Ontario Regional Championship Show & Breed Survey Form.

Reminder - deadline date for submitted show entries is
May 21, 2010.

All information regarding supporting documentation required for entry submission is provided on each form, please review carefully.

Please make all cheques payable to "Durham Schutzhund Club" and forward your entry with photocopies of your dog's registration to:
